What is a case manager and what do they do?

Case managers play a crucial role in connecting clients with the social services they need. They act as the liaison between you and organizations, businesses or professionals that provide solutions to get back or stay on track.

First, a case manager will listen to your story and ask you a few questions about your life goals and dreams. You’ll spend some time discussing the current barriers and challenges you face and help you develop a plan to overcome them. Together, you’ll establish a road map to success and determine what next steps you should take.
